According to Centers for Medicare & Medicaid Services, lack of proper care transition is one of the major causes for re-admission to the hospital within 30 days of discharge.

Flornce™ is mHealthCoach’s machine learning powered smart care platform for patients, and helps keep them on track with their post-op regimens to reduce re-admittance and achieve higher adherence.

With Flornce™, patients are never alone, the machine learning module is able to personalize care and understand non-adherence pattern from its libaray of vast data sets as well as newly created learning units.

Flornce™ lets caregivers input medication protocols using the single-source mHealthCoach Medication Adherence robust scheduling tool that sends reminders about when to take medicine, when to check for side effects, and when upcoming refills are due.

Flornce™ also facilitates comprehensive messaging between all interested parties. Primary care physicians, specialists, physical therapists, and home care nurses can all participate in a patient’s recovery by exchanging messages, appointments, reminders and calendar synching.

In addition to the caregiver team, Flornce™ lets family members with a Web connection anywhere in the world actively participate in the patient’s recovery.

Flornce™ has a face and a voice for delivery of weekly assessment progress reports and propose intervention steps. Bi-lingual ability is provided for higher patient engagement.
